How to Organize Medical Information
Simple, low-tech ways to keep appointments, providers, and questions organized, without storing sensitive records in the wrong places.

When several providers and appointments are involved, information scatters quickly. You don't need a complicated system, just a consistent one.
Start with a one-page summary
Create a single page that lists:
- your providers, with phone numbers and what each one helps with
- upcoming appointments
- the questions you want to ask next
- the pharmacies you use
Keep it where you can find it, and update it after each visit.
Keep a running notes page
After each appointment, write down the date, who you saw, what you were told, and what happens next. Short notes are far more useful than none.
Use the patient portal wisely
Many health systems offer patient portals where you can see visit summaries and message your care team. Ask each office how to sign up and whether a family member can be given access.
Be thoughtful about where you store sensitive information
Health information is personal. Prefer secure, password-protected storage you control, and be careful about sharing it by regular email, text, or public forms. Websites like ours do not want sensitive details through general inquiry forms.
Review regularly
Once a month, look over your summary page and notes to see what's changed and what questions are still open.
